- Insurance cards and drivers license need to be presented at the time of check-in for your visit
- Co-Pays are due at the time of service.
- We are a NO SMOKING FACILITY
- If you are unable to keep your appointment, kindly give 24 hours’ notice.
- If you have a surgery scheduled and need to reschedule, please give at least 24 hours’ notice
